I have turned in as many as (10) sick call slips in the past (30) days. What are you people doing with them?
In MAX #5 the sick call box sits in the floor with no lock on the ...[indistinguishable word]; your inmate porters use the sick call box as a "chair." I have a large lump under my right breast nipple causing me severe problems-plus other problems.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
I want to receive proper medical treatment for #1: lump/knot that is under my right breast nipple. #2 Treatment for my right arm-it turns purple for no reason.#3 I can barely see at all out of my left eye-cannot read.

I have put in as many as (10) sick call slips-your ...[indistinguishable word] for sick call slips sits in the floor-has no lock on the ... [indistinguishable word] and it is used as a chair more than anything else. I don't know where the slips are going. This is my 2nd grievance on my need for medical treatment. Ignoring my medical needs and/or denying me access to treatment is cruel and inhuman.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
To receive proper medical treatment. I will specify to help you: #1 my left eye is losing all focus, I cannot read with it period. #2 Lump/knot under right breast nipple-painful pressure. #3 Right arm turns purple without cause.

On May 24, 2003 my internal bleeding started up again. I am formally requesting a colonoscopy to determine if I have cancer or what the cause is. I have a lump/knot (possibly a tumor) under right breast nipple causing odd pressure, pain in chest area. My right arm will turn purple for no known reason. My left eye has convulsive spasms rendering extremely blurred vision problems.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
Much of this has been ongoing for several years with no adequate treatment as of yet. Portions of it started up in the past 6 to 8 months and the sick call up process doesn't work. I want immediate treatment.

Give a detailed reason for request We do not necessarily need to talk, but-I am being forced to use a request form that I do not have and cannot seem to find so that I can request other forms. What I need and want are-multiple grievances and multiple sick call slips, can you handle this? This needless process is delaying my medical request! Why do you have inmates in control of medical forms and in charge and control of emergency grievance forms? Explain?
